Assessment Counselor

Should have one (1) year or more prior experience working with drug dependent persons and have knowledge in the areas of chemical dependency, including causes, effects and treatment prevention programs. Must be LCDC III, LICDC, LPC, LSW, LPCC or LISW level. B.A. or B.S. degree in Social Service field preferred.

DUTIES / RESPONSIBILITIES                           

 1.  Evaluates Consumer’s specific problems/co-disabilities and develops treatment plans for each problem.

2.   Provides Substance Use Disorder Diagnostic assessment within scope of practice and least restrictive level of care based upon ASAM Level of Care Dimensions.

3.       Provides referral services both internal and external (i.e., Employment/Education/Vocational rehabilitation services).

4.       Performs outreach and follow-up services for active and inactive clients.

5.       Prepares consumer progress notes, daily MIS forms, discharge forms, and other reports as needed.

6.       Performs SUD Counselor Duties and is assigned counseling case load.

7.       Provides community drug prevention and information services.

8.       Prepares intake/admission record on applicants.

9.       Review OAARS and Central Registry documentation, discharge documentation from losing agencies then prepare letter for admission   review for PCI physicians on consumers who have been in another medication assisted treatment program or requesting to transfer to   this agency.

10.     Gathers consumer data and screening information on initial call/referral.

11.     Participates on Quality Assurance Committee as assigned.

12.     Performs Case Management as needed.

13.     Maintains a minimum caseload of 24 consumers. 

14.     Provides individual and group counseling for consumers based on their Individualized Service

          Plan and recommended Level of Care.

15.     Prepares consumer progress notes, group notes, counselor’s log, group notes, consumer’s

          appointment, discharge forms, and other reports as needed in EHR.

16.    Prepares charts for review during Case Conference, and Maintains record in compliance with

         all applicable standards.

17.    Completes a 90-day outpatient review of client charts on counselor’s caseload and makes

         report to supervisor.

18.    Maintains a professional manner and ethics on the job.
