Case Manager

Associate Degree in Social Services Field/Case Management with relevant work experience, Certified or eligible for licensure by the Ohio Credentialing Board or Counselor and Social Worker Board. LCDCII or LSW with scope of practice in Chemical Dependency preferred, must be experienced in the operation and understanding of computers. 1. Must have good public relations. Ability to establish and maintain a positive, effective working relationship with community groups, agencies, and other employees. a. Considerable knowledge of case management functions. b. Considerable knowledge of community organization skills. c. Must possess a high degree of initiative. d. Strong desire to help others, patience understanding, strong sense of responsibility. e. Able to manage time effectively. f. Must have thorough knowledge of computer software and hardware. Must have ability to handle queries and hardware integration.

DUTIES / RESPONSIBILITIES

       1.           Assesses consumer needs and identifies the appropriate case management service for the consumer and family.

2.           Coordinates referral services to assist consumers in obtaining necessary services, transportation, financial, housing, food, clothing, medical, educational, vocational, employment, legal medical/mental health, aftercare services, and others deemed appropriate to meet their needs.

3.           Assists consumers and/or family to utilize referral, and determines adequacy of services provided.

4.           Provide case management follow-up by making telephone contact to ensure services are being received.

5.           Review records, and document case files.

6.           Prepares reports as assigned.

7.           Participate in Staff Meetings, Case Conference Team, QA/QI Meetings, and Safety Committee.

8.           Creates the necessary files and databases for data management.

9.           Enter Initials Contact Forms / MACSIS Behavioral Health Data

10.        Maintains a minimum caseload of 24 consumers. 

11.        Provides individual and group counseling for consumers based on their Individualized Service Plan and recommended Level of Care.

12.        Prepares consumer progress notes, group notes, counselor’s log, group notes, consumer’s appointment, discharge forms, and other reports as needed in EHR.

13.        Prepares charts for review during Case Conference, and Maintains record in compliance with all applicable standards.

14.        Completes a 90-day outpatient review of client charts on counselor’s caseload and makes report to supervisor.

15.        All other duties as assigned by supervisor.
