Intensive Outpatient/Women Into A Lasting Lifestyle Chemical Dependency Counselor

BA/BS in Social Service field preferred. Must be LCDC II or LCDC III. B.A. or B.S. degree in Social Service field preferred. Candidate should have at least one year working with drug dependent persons or in a social service field/agency. Good writing and oral communication skills required. DUTIES / RESPONSIBILITIES

 

1.       Counsels clients on an individual and group basis.  Evaluates and documents

          progress of consumer.

2.       Writes and implements individualized treatment plans, based on client needs.

          Conducts treatment plan reviews in accordance with Local, State and Federal

          Regulatory guidelines.

3.       Conducts group therapy and educational classes.

4.       Provides Referral and Case Management services, (i.e., social services, educational

          services, vocational services, etc.)

5.       Participates in home visits.

6.       Conducts screening, intake, and assessments.

7.       Monitors client urine samples, prepares urine service tickets, and documents urine

          log book.

8.       Assists in identifying programming needs and new treatment approaches through

          consultation and group discussions with other staff members.

9.       Compiles case records and other data.  Performs record keeping in accordance with

          Local, State and Federal Regulatory guidelines.

10.     Prepares reports as required.

11.     Provides community drug prevention and information services.

12.     Performs follow-up services for active and inactive clients and other related work as may

          be required.  Participates in staff meetings, case conferences, Quality Assurance

          Improvement Activities, and In-Service Training.

 13.     Maintains professional skills level in accordance with certification standards to

          ensure the performance of quality and efficient care is being provided to the

          consumer.

14.     Performs other duties as assigned by supervisor.
