Outpatient Chemical Dependency Counselor

BA/BS in Social Service field preferred. Must be LCDCII or higher. Candidate should have at least one year working with drug dependent persons or in a social service field/agency. We provide excellent benefit package and a supportive work environment.

DUTIES / RESPONSIBILITIES

               

1.       Evaluates Consumer’s specific problems/disabilities and develops treatment plans for

          each problem.

2.       Provides individual and group counseling for clients based on their Individualized Service Plan and recommended Level of Care.

3.       Provides referral services (i.e., Social Services, Educational Services, Vocational training,

          etc.).

4.       Performs outreach and follow-up services for active and inactive clients.

5.       Prepares client progress notes, daily MIS forms, group notes, counselor’s log, group

          notes, consumer’s appointment calendar, discharge forms, and other reports as needed.

6.       Monitors client urine samples, prepares urine service tickets, and documents urine

          log book.

7.       Provides community drug prevention and information services.

8.       Prepares intake/admission record on applicants. Provides consultation and group  discussions with other staff members.

10.     Participates on Quality Assurance Committee as assigned.

11.     Performs Case Management as needed.

12.     Prepares charts for review during Case Conference, and Maintains record in compliance with all applicable standards.

13.     Completes a 90-day outpatient review of client charts on counselor’s caseload and makes

          report to supervisor.

14.     Maintains a professional manner and ethics on the job.

15.     All other duties as assigned by Supervisor.

16.       Monitors, collects and organize the data for each P.C.I. quality indicator and ODADAS Required Outcome Measures.
