Outpatient Counselor

in Counselors

BA/BS in Social Service field preferred. Must be LCDC II, III, LPC, LSW, LPCC or LISW. Candidate should have at least one year working with drug dependent persons or in a social service field/agency. We provide excellent benefit package and a supportive work environment. May also consider CDCA with 1yr. experience.


  • Evaluates Consumer’s specific problems/disabilities and develops treatment plans for each problem.
  • Provides individual and group counseling for clients based on their Individualized Service Plan and recommended Level of Care.
  • Provides referral services (i.e., Social Services, Educational Services, Vocational training,etc.).
  • Performs outreach and follow-up services for active and inactive clients.
  • Prepares client progress notes, daily MIS forms, group notes, counselor’s log, group notes, consumer’s appointment calendar, discharge forms, and other reports as needed.
  • Monitors client urine samples, prepares urine service tickets, and documents urine log book.
  • Provides community drug prevention and information services.
  • Prepares intake/admission record on applicants. Provides consultation and group  discussions with other staff members.
  • Participates on Quality Assurance Committee as assigned.
  • Performs Case Management as needed.
  • Prepares charts for review during Case Conference, and Maintains record in compliance with all applicable standards.
  • Completes a 90-day outpatient review of client charts on counselor’s caseload and makes report to supervisor.
  • Maintains a professional manner and ethics on the job.
  • Monitors, collects and organize the data for each P.C.I. quality indicator and ODADAS Required Outcome Measures.
  • All other duties as assigned by Supervisor.


  • Should have one (1) year or more prior experience working with drug dependent persons and have knowledge in the areas of chemical dependency, including causes, effects and treatment prevent programs.


  • Must be, LCDC II, III, LPC, LSW, LPCC or LISW level.  B.A. or B.S. degree in Social Service field preferred.
